    A, the answer given is the correct one. PRINCE2 defines 3 types of issues. 1.Issues, 2.Request for Change, and 3.Off-Specifications. The Issue Report is updated with the outcome of the final decision on its contents. The Change Management Strategy in general will dictate how, and to whom to submit an RFC, option 4 is not a function of an Issue Report.

    I think that the correct answer is B. Please see the extract from Appendix A from Managing successful projects with Prince2 (Section A. 13. 1 Purpose):
    “An Issue Report is a report containing the description, impact assessment and recommendations for a request for change, offspecification or a problem/concern. It is only created for those issues that need to be handled formally.”
