Redundancy designs (answer B)attempt to eliminate single points of failure, where one failed device or design element brings down service.
Techniques (answer D) exist to detect failure and trigger failover to a redundant device.
With proper failover, services should continue operating when single components fail. Without component monitoring, a failure to detect and replace a failed redundant component can lead to an outage when the second component subsequently fails.
Network diagrams help in planning and in fixing outages more quickly.
But question was “minimize the effect of outages”, then answers B and D are not right.
I think , that right answers are C and E.

If you were to HAVE an outage, it means that your well planned redundant $10Mil equipment has already failed. Say your best engineers are out of the country and the backbone is down,
b) the redundancy will no longer help you when it is broken/down any faster
d) the best gear possible, when broken/down, will not solve the problem any faster
e) monitoring and reporting tools won’t help you fix a problem you already know exists any faster
The way such an outage gets resolved fastest is highly dependent on (a) the staff communication and skills and (c) documentation, change control, and labs. Therefore, in my opinion, the best answers are A and C.
