B, C are correct. dvSwitch can be created with one uplink and an ESX server cannot create or configure a dvSwitch by itself.

C. Because Distributed Switch is vCenter feature, ESXi host must be added to vCenter for vCenter to publish the Distributed Switch to the ESXi host.
