Should be only 1 answer per column (the answer above is wrong). Note that the question doesn’t state what the Forest functional level is, and thus we can’t assume it needs to be raised to Server 2008.
Agree though that ‘assigning licenses’ is the only activity in the list that would be considered a Migration task, and not a Planning task.

I think only the first two options should be checked. Forest functional leven – 2003 would be ok, 2008 isn’t necessary. And the question doesn’t say anything about the current level.
Migration to MX2013 isn’t necessary, too. We run a hybrid exchange and it’s still 2010. Irritating question.

1 – Assign Licenses to users
Prepare Phase has:
2 – Prepare your on-premises Active Directory for directory synchronization.
Exchange 2010 is compatible with Office 365 hybrid. https://technet.microsoft.com/en-us/library/hh563847(v=exchg.141).aspx so 4 isn’t required, or one of these answers.
Forest Functional Level may need to be raised (if it’s below 2003 FFL), but raising the FFL isn’t listed verbatim in the guide. I couldn’t find any more information on the three phases of the fast track either. Plan phase does list “Identify Active Directory preparation and remediation requirements”, and this “could” cover upgrading the FFL. So I’m leaning more toward this being the correct answer.
So for Migration phase 1 is correct for sure.
As for the Planning phase it’s 3 in my opinion but it could possible be 2.
2 is actually listed in the guide almost verbatim and 3 isn’t, and maybe it was just a typo putting 2 in the Planning Phase rather than the Prepare Phase.
I couldn’t find a clear cut answer for this one though sorry.
