FT heartbeats actually occur between the ESX hosts and not the individual VMs. The only time the FT secondary will automatically take over for a failed primary is if the ESX host hosting the primary fails. The primary ESX host failure is detected by the secondary ESX host when it sees a loss of heartbeating. In the case where the ESX host is running and the individual VM fails (i.e. hangs, blue screens, becomes corupt), the heartbeats between ESX hosts will continue, there will be no takeover, and the FT record-replay logic will simply replicate the hang/blue screen/coruption to the FT secondary.
